I recieved an email today saying my subscription expires in 90 days. Do i need to renew? Is the license perpetual? If the subscription is just for help and technical assistance than I do not need the subscription. In my licenses section under my account I see perpetual at the top with my current designated computer license.

The email you received should be explicit about what happens if you allow the subscription to lapse. For example, mine says
==========================================================================================
Renew your subscription before the Service End Date to ensure your team has uninterrupted access to:
  • The latest updates to your MATLAB and Simulink products – with releases twice a year
  • Technical support available in 10 languages via telephone, email, and the web
  • Online offerings such as MATLAB Online, MATLAB Drive, and MATLAB Mobile
  • Comprehensive license management and the ability to purchase add-on products
==========================================================================================
MATLAB should not stop working. As you say, the ability to run MATLAB is perpetual. But you may lose more than just technical support.
All that being said, for absolute certainty for your case, you should contact the sales team, using the link in the email you received.
