How to read signal/data value from simulink rea-time model, on App designer ?

I have a simulink model that reads signals from a NI-DAQ through the "Analog input" and "Digital Input" blocks in "real time", by this I mean putting "inf" in simulation time. With App Designer I can send data but I still don't know how to receive it.
