The website did not answer these questions, nor did the help desk
1) Do students have to go to a center for proctoring or can they take it proctored from home?
2) If they can take it from home, can they take it pretty much "immediately"?
3) If they have to go to a center, can they take it within 2 weeks? 4 weeks? of registering
4) Is there a way for me to make sure the certificate is legit? a public registry? if there there is a public registry, do they have to opt in?

Based on the statement on this page that the Certified MATLAB Professional exam is delivered "at any MathWorks owned facility" I believe the answer to your first question is that you have to go to a center for proctoring. This renders your second question moot.
For your third question I'm not sure what the limit is for how far in advance of the exam date you're allowed to register (I mean I doubt you could schedule a session for 5 years from now) but the certification program policies linked on that page state "Registration for certification exams at MathWorks facilities closes 24 hours prior to the exam, and candidates who cancel their exam forfeit their exam fee. Please direct any questions to training@mathworks.com." There's also a button on that page that lets you view upcoming certification exams, so you can see what dates are available right now.
I don't know the answer to your fourth question so I'd suggest contacting the Training department directly. If you want additional certainty on your first question you could ask Training that question as well.
