Controller pins mapping with Simulink in/out ports

Hello all, how can i map the Simulink inports and outports with the controller pins. Means if i want to read some sensor value (after code generation for some controller) and after some manipulation send that to Display, so how will i read and write the value on target controller physical pin (by code generated by simulink for that target)? Thanks
