How to make a GUI (guide) figure

I would like to make a GUI figure of the following script, but as I have never made a one before, don't know where to start.
I would like the GUI to look something like this.
Any help is appreciated. Thanks in advanced.

The Matlab help is always the best place to start, e.g.
doc guide
and follow the links in 'Topics'
