This is a pretty big question. Is there a particular algorithm that you are trying to implement? A simple Google search for "Matlab adaptive thresholding" yields a bunch of results--you could just try one of those.

that depends on a lot of things, but you could try using im2col to analyze your image in discrete blocks and apply whatever function you want to get the binary result. You could also do like three different convolutions with different kernels and use the residuals to index which result should construct what part of the final binary result. You could also use expectation maximization to guess at the proper threshold for a section of the image. Or you could use a level set method or graph cut to segment your image. what type of image do you want to threshold? that would help you chose the code.
