Use dicominfo to determine the x and y separation between pixels. Once you have that you can use meshgrid or ndgrid to find the x and y coordinates.
After that, if you have a single slice, use the intensity as the z coordinates.
If you have multiple slices then figuring out the z coordinates can depend on the modality and avaliable headers. I would have to review the various possibilities (I have posted them in the past.)

DICOM is a standard for representing medical images, including header information about the image such as patient identification, dates, frequency , positions . Different branches of medicine have quite different image requirements, so dicom needs to know which branch of medicine is being used so that it knows which headers to put in. The header information can be retrieved using dicominfo().
The way that positions are described in headers depends upon the branch of medicine, so in order for me to tell you which headers to examine you need to tell us which branch of medicine the image is. For example , is it an opthomology image ? Is it PET? Is it slide microscopy?
