A problem in working with Nonlinear MPC

Hello, I ran a program with NMPC block in Simulink, but it makes some problems that I do not understand why. Could you please help me what are they and how to fix it. The errors are:
Error using EMPC1_CSTR (line 132)
Error due to multiple causes.
Caused by:
1- Error using EMPC1_CSTR (line 132)
When parameter 'InputProcessing' is set to 'Elements as channels (sample based)' and data input signal is 2-D or N-D signals, the dimension of initial condition should
be scalar or equal to the dimension of data input if it is unit delay.
I do not understand this error.
2- Error using EMPC1_CSTR (line 132)
Error in port widths or dimensions. Output port 1 of 'untitled/Nonlinear MPC Controller/xmvs_router/Product1' is a [16x4] matrix.
I defined just two initial states in my program, but it says I have 4.
