Send data from Arduino ethernet shield to Simulink

I have to send a float data (read from a sensor) from Arduino to Simulink using Ethernet shield. This data has to be plotted on Simulink.Is there any clear and concise example that show the code of Arduino for doing this and the model&configuration of Simulink ?
